Wegovy Face: What It Is and Why It Happens

Are you worried about developing "Wegovy face" during your medical weight loss journey? This term describes the hollow or sagging facial appearance that can occur after rapid weight loss. It is a natural result of losing subcutaneous fat, not a direct side effect of the medication itself. Discover why your face changes during weight loss and how you can support skin health.

What Is “Wegovy Face”?

"Wegovy face" – often also referred to as "Ozempic face" – is a widely used, non-medical term that has gained significant attention in media and on social networks. It is used to describe noticeable changes in a person's facial appearance after they have lost a significant amount of weight using modern GLP-1 receptor agonists, which are weight loss medications like Wegovy and Ozempic.

People experiencing this phenomenon typically report that their face looks more hollow, gaunt, or aged. The primary characteristics include more prominent cheekbones, sunken eyes, increased wrinkles, and loose or sagging skin around the jawline and neck. It is important to note that "Wegovy face" is not a clinical diagnosis or a medical condition; it is simply a descriptive term for the physical signs of facial volume loss and fat reduction.

Why Can Weight Loss Change Your Face?

To understand why your face changes, it is important to consider how your body stores fat. The face relies on subcutaneous fat (the fat stored just beneath the skin) to maintain a smooth, plump, and youthful appearance. This fat acts like structural scaffolding, keeping the skin taut and lifting the facial tissues, contributing to overall facial volume and structure.

When you are in a calorie deficit and begin to lose weight, your body burns fat globally. You cannot dictate where you lose fat first. As you lose excess visceral fat around your organs, you also lose subcutaneous fat across your entire body – including your face.

When the underlying fat pads in the cheeks and jaw shrink, the skin loses its structural support. If the skin's elasticity is compromised due to age, lifestyle factors such as sun exposure, or prolonged stretching, it may not shrink back perfectly to fit the new, smaller facial structure. This loss of volume and support is what creates the hollow or sagging appearance and more prominent facial contours.

Is Wegovy the Cause of “Wegovy Face”?

A common misconception is that the medication directly affects facial tissue. However, Wegovy (semaglutide) is not the direct cause of these changes.

The active ingredient in Wegovy works by mimicking a naturally occurring hormone (GLP-1) that regulates appetite and slows down gastric emptying. This helps reduce energy intake and facilitates a sustained calorie deficit. The facial changes are primarily a result of the rapid and substantial weight loss itself, not the pharmacology of the drug.

In fact, the exact same facial changes frequently occur in patients who lose large amounts of weight through bariatric surgery or non-surgical medical weight loss alternatives. The reason the term is linked to Wegovy is simply that these modern weight loss drugs are highly effective, allowing a growing number of people to achieve profound weight loss relatively quickly.

How Common Are Facial Changes During Weight Loss?

Facial changes are common and are a normal biological response to significant weight reduction. However, the severity of "Wegovy face" varies greatly from person to person.

Several factors determine how noticeable the changes will be:

  • The amount of weight lost: Losing 20 kilograms will naturally have a much more visible impact on your facial volume than losing 5 kilograms.
  • The speed of weight loss: Losing weight quickly gives your skin less time to adapt and retract, increasing the chance of loose skin and sagging facial skin.
  • Age: As we age, our bodies produce less collagen and elastin, the proteins responsible for skin elasticity. Older skin has a harder time shrinking back after the underlying fat is gone, which can lead to an aged appearance.
  • Genetics: Your genetic makeup largely dictates your skin's natural elasticity and where your body stores (and loses) fat. Some people naturally carry more facial fat, which affects how their face changes during weight loss.

Ways to Support Skin and Facial Appearance During Weight Loss

While you cannot stop your body from losing facial fat if you are losing weight globally, you can take strategic steps to support your skin's health and minimize sagging.

  • Pace your weight loss: The most effective strategy is to avoid "crash dieting." By adjusting your medication to support a steady pace and lose weight gradually, you may allow the skin more time to adapt to changes in facial volume.
  • Prioritize protein and a balanced diet: Protein is essential for preserving lean muscle mass and supporting the production of collagen. Eating lean meats and healthy fats, along with vitamin- and mineral-rich foods support overall metabolic and skin health. A balanced diet provides the building blocks your skin needs to maintain its strength and elasticity.
  • Stay hydrated: Dehydration makes wrinkles and hollow areas look much more pronounced. Drinking plenty of water may improve skin appearance and well-being.
  • Protect your skin: Sun damage destroys collagen and accelerates skin aging. Using daily sun protection with SPF and a good moisturizer supports overall skin health.
  • Incorporate physical activity: Both resistance training and cardiovascular exercise play important roles during weight loss. Resistance training such as lifting weights or bodyweight exercises helps maintain muscle mass during weight loss. Cardiovascular exercise improves heart and vascular health, increases oxygen uptake, and strengthens lung function. It also supports better blood sugar control, reduces the risk of cardiovascular disease, and can improve energy levels, sleep, and mental well-being. All of the body’s tissues, including the skin, are positively affected by physical activity.
  • Maintain healthy lifestyle changes: Sustainable weight loss combined with healthy lifestyle changes supports metabolic health and overall well-being, and may reduce cosmetic concerns.

Can Facial Changes Improve Over Time?

Yes, for many people, the appearance of "Wegovy face" improves over time. Skin is a living organ, and it possesses a certain degree of natural elasticity. Once your weight stabilizes in the maintenance phase, the skin will gradually contract and adapt to your new facial contours.

However, it is important to have realistic expectations. If you have lost a large amount of weight or are older, the skin may not fully retract on its own. In these cases, some individuals choose to consult with a dermatologist or aesthetic specialist to explore medical treatments or cosmetic treatments that stimulate collagen production or restore lost volume.

Remember, changes to your facial fat are a small cosmetic trade-off for the profound health benefits of treating obesity. By reaching a healthier body weight, you significantly reduce your risk of cardiovascular disease, type 2 diabetes, and severe joint pain – adding healthy, active years to your life.

If you feel self-conscious about facial changes during your weight loss journey, consider seeking ongoing support from a healthcare provider or counselor to address body image concerns, support psychological well-being, and explore individualized, medically supervised weight loss support.

Every second, your body uses energy. Your brain needs fuel to think, your heart to keep beating, and your lungs to fill with air. Even when you are lying still, your body is constantly working in the background. All of this requires fuel. Metabolism, also known as metabolic rate, is the process by which the body converts food into energy.
